What if I have an emergency?
The sudden onset of a condition that results in pain and medical distress may require urgent attention. In the case of an emergency, call 911 or go to the nearest emergency room. Be sure that you or your caregiver shows your Broward PACE Program ID card and contacts us for immediate authorization. If you feel you need healthcare services, but are not experiencing an actual emergency, contact your Broward PACE Program team.
Who are my doctors?
When you enroll in PACE, you receive care from our medical team. The PACE Medical Director will be your primary care physician. For any specialty care needed, you will be referred to a contracted specialist. In addition to your primary care physician, you will have a full clinical team at your disposal.
Does PACE provide transportation?
PACE drivers ensure safe door-to-door travel between a participant’s home and the PACE center, as well as other health care appointments.
When is Broward PACE Program open?
We are available 24/7 to help our participants with urgent care needs. Our Adult Day Health Centers are open from 8am – 4pm, Monday through Friday. Participants can visit during those hours for breakfast, lunch, activities, and coordinated primary and specialty care.
Participant Access, Rights & Responsibilities
Participant Access to Services and Providers
Upon enrollment, Broward PACE Program is the sole service provider. Participants are guaranteed access to services, but not to a specific provider, and will have access to all medically necessary care as determined by your primary care provider and your care team. This means that the services you receive are needed to prevent, diagnose, correct, or cure any conditions that cause acute suffering, are life threatening, result in illness, interfere with daily living, or threaten some significant disability. When a participant needs to access covered services, their care team will select from the Broward PACE Program provider list and help make the arrangements, including transportation, so the participant receives the needed care. Participants who are dissatisfied with specific providers should call their care team and request a change. The care team will select a new provider in time for the next scheduled or requested appointment. Our provider list is available upon request.

Your Rights Upon Disenrollment
As a participant, you may disenroll from Broward PACE Program at any time, with proper notice. Your benefits from Broward PACE Program can be stopped if you choose to cancel your enrollment in the program. If you elect to cancel these benefits, your decision is a voluntary disenrollment. If you no longer meet the requirements for enrollment in the program, you will lose the benefits through an involuntary disenrollment. Broward PACE Program will provide you with written notice in the event of an involuntary disenrollment.
Enrolling with Broward PACE Program results in disenrollment from any other Medicare or Medicaid prepayment plan or optional benefit. Electing enrollment in any other Medicare or Medicaid prepayment plan or optional benefit, including the hospice benefit, after enrolling as a Broward PACE Program participant is considered a voluntary disenrollment from Broward PACE Program. If a Medicaid only or private pay participant becomes eligible for Medicare after enrollment in Broward PACE Program, the participant will be voluntarily disenrolled from Broward PACE Program if he or she elects to obtain Medicare coverage other than Broward PACE Program.
Voluntary disenrollment will be effective on the first day of the month following the date Broward PACE Program receives the participant’s notice of voluntary disenrollment. However, if the notice is received near the end of the month, the disenrollment may not be effective until the following month. This is due to the time it takes to coordinate payment termination between Medicaid and Medicare.
Involuntary disenrollment will be effective the first day of the next month that begins 30 days after the day Broward PACE Program sent you a written notice of the disenrollment.
Broward PACE Program will work with Medicare (Centers for Medicare and Medicaid or “CMS”) and Medicaid (Agency for Health Care Administration or “AHCA”) to help facilitate the participant’s transition and reinstatement in other Medicare and Medicaid programs for which the participant is eligible. This coordination will include making appropriate referrals and ensuring medical records are made available to new providers within 30 days.
Until enrollment is terminated, Broward PACE Program participants are required to continue to use Broward PACE Program provider services and remain liable for any premiums. Broward PACE Program shall continue to provide all approved services until the date of disenrollment. Broward PACE Program will not be liable for non-approved services with the exception of emergency services.
