How to Enroll in PACE

Broward PACE Program was created to provide essential services exclusively to older adults in Broward County. We invite all eligible community members age 55+ to find out more.

Get Started Today

Step 1.

Pre-Screening

First, call 954.787.0191 or TTY 800.955.8771 to learn if you are eligible for PACE. Our enrollment team will explain the process and what to expect. We can also help you enroll in Medicaid if needed.

Step 2.

In-Home Visits

Next, we review the enrollment paperwork with you and your family in your home. We’ll talk about your needs for healthcare, socialization, and other PACE services. Since the state administers PACE, you will be contacted by the Florida Department of Elder Affairs for a level of care assessment.

Step 3.

Application Processing

The entire process takes about 90 days. During that time, you can tour Broward PACE Program and meet your new care team. We’ll answer whatever questions you have while your application is being processed.

Step 4.

Customized Care Plan

Once you’re enrolled, we will work with you and your family to build a unique care plan for your personal health, social, and quality of life goals. Our services evolve along with your needs. There are no worries – your PACE team will manage and coordinate everything for you.
