This is a time to recognize the profound impact of the Program of All-Inclusive Care for the Elderly (PACE), an innovative model of care that empowers seniors to live dignified, independent lives in their own homes and communities.
The theme for 2025 is “Your Health. Your Journey. Our Support.” This perfectly encapsulates our mission at Broward PACE Program. We understand that every individual’s healthcare journey is unique, and we are committed to providing personalized, comprehensive support every step of the way.
What is PACE?
For those unfamiliar with PACE, it is a comprehensive healthcare program for adults aged 55 and up who require nursing home-level care but wish to remain in their community. At Broward PACE Program, we provide PACE participants with a wide range of services including primary care, specialty care, physical and occupational therapy, prescription drugs, transportation, and adult day care with meals and social activities. Our interdisciplinary team works with each participant and their family to create a customized care plan that addresses their specific needs and goals.

The PACE model has proven to be effective at enhancing the lives of older adults. PACE participants are more likely to live longer, and their rates of hospitalization and institutionalization are reduced.
