PACE – Program of All-Inclusive Care for the Elderly – provides personalized healthcare and so much more to seniors in central and northern Broward County. Broward PACE Program is a collaboration between Broward Health and Florida PACE Centers by Miami Jewish Health. You may qualify for Broward PACE Program if you:
Keeping You Healthy as You Age
Everyone has different health priorities, social goals, and preferences about receiving care. Broward PACE Program will work with you and your family members to create an individualized care plan to maintain your health and quality of life.
The PACE team will coordinate all your appointments, medications, and care needs. We also offer Adult Day Care, including transportation to and from the center, meals and snacks, and activities as part of your enrollment. Your PACE team will include:
- Primary care physician
- Medical specialists
- Dentist
- Nurses
- Physical therapist
- Occupational therapist
- Recreational therapist
- Social worker
- Dietitian
- Home care coordinator
- Site director
- Shuttle bus driver
Program Costs
If you are enrolled in Medicaid, all your costs may be covered and your out-of-pocket expenses could be:
$0
If you are enrolled in Medicare and Medicaid, all your costs may be covered and your out-of-pocket expenses could be:
$0
If you are enrolled in Medicare only, you will pay a monthly premium to cover some expenses. If you don’t have Medicare or Medicaid, you can pay for PACE privately.
